tim+: The New Messenger Service for the German Healthcare Sector

Finding a secure messenger service that meets the requirements of the GDPR and the DSK (2021) has not been easy for healthcare participants. Probably the most challenging task is finding a messenger service that is not only secure but also interoperable. 

 

To close this gap precisely, healthcare experts launched the idea of a secure and interoperable messenger service for the German healthcare sector in 2019. What was initially just an idea was enshrined in law in 2021 with the Digital Supply and Care Modernization Act (DVPMG). The time has finally come: All healthcare providers in the German healthcare system can use a secure and interoperable TI messenger to chat with their colleagues inside and outside their organization and exchange necessary patient data such as medication plans, blood, or laboratory values. 
 

Proven Quality and Security

Users do not have to rely on the manufacturer's specifications. The TI messenger service was developed using gematik specifications and tested by certified experts for interoperability, security, and data protection. The TI messenger thus bears a seal of approval, unlike any other messenger service. Anyone using a TI messenger can be sure it is DSGVO, DSK compliant, and compatible with other TI messengers. In addition to intersectoral and interoperable communication, TI Messenger also paves the way to TI 2.0 and is not only more flexible than other TI applications but can be integrated into the practice management system (PVS), used as a standalone solution, and installed on the desktop or mobile phone.

tim+ enables the creation of individual and group chats, also with the involvement of patients. Communication can be case-based, which supports structured and context-related collaboration. Functions such as read and send confirmations, including private read confirmations, ensure transparency in the message history. An integrated presence indicator shows the availability of contacts. Frequently used contacts or rooms can be marked as favorites, while individual rooms can be muted if required. Searching in the organization and global address directory is intuitive. There is also a reporting function that can be used to report messages.

The exchange of documents (e.g., Word, Excel, PDF) as well as images and other digital content is possible up to a size of 100 MB. In addition, tim+ offers an export function that can be used to save and process both chats and files.

A powerful Org Admin Client enables the central administration of users and roles. The platform is capable of being multi-client and, therefore, supports various institutions within an organization. The assignment of rights is role-based. Broadcasting messages and server notifications can also be sent. Device management is kept simple, and organizational accounts and personnel assignments can be created efficiently. Health care services and areas can also be set up. The connection of chatbots is possible, as is the operation of several messenger services within an organization.

tim+ relies on the highest security standards: End-to-end encryption (E2EE) takes place directly on the device. Secure two-factor authentication (2FA) is integrated, and other authentication types are supported. Functions such as cross-signing and Secure Secret Storage and Sharing (SSSS) further increase security. The solution is fully GDPR and DSK-compliant and meets gematik's interoperability requirements. Data protection information is recognizable, and it is possible to configure deadlines for automatically deleting data and chat rooms. An automatic logout after inactivity provides additional protection against unauthorized access.

tim+ is operated exclusively in Europe - preferably in Germany - which offers additional security regarding data protection and compliance.

Thanks to open interfaces (API), tim+ can be seamlessly integrated into existing systems such as practice management systems (PMS) or hospital information systems (HIS). Modern authentication methods such as OpenID and Single Sign-On (SSO) are also supported.

Use Cases of tim+

  • Julia works in a nursing home. When she discovers an infected wound on a resident, she acts immediately. Using the TI Messenger, she takes a photo, writes a short message, and sends both directly to the responsible GP - no waiting on hold, no fax machine. Ten minutes later, she receives a reply with a clear recommendation for further care. The resident can stay in the home - the situation is under control.

     

    Advantages: Quick consultation with doctors enables rapid care and reduces unnecessary hospital admissions. At the same time, communication is documented and can be incorporated directly into care reports.

  • A complex diagnosis is pending at the hospital. Dr. Nguyen, a senior physician in internal medicine, starts a secure messenger chat with colleagues from radiology and oncology. MRI images, lab results, and voice messages are shared in real time. The treatment plan is ready within an hour, without a conference room, without paper files, but with bundled expertise.

     

    Advantages: Interdisciplinary collaboration is made possible in real time. The messenger can be integrated into existing hospital systems (HIS), enabling the secure exchange of sensitive medical data.

  • Dr. Masri is a general practitioner in a rural area. Her patient, Mrs. Klein, is 84 years old, chronically ill, and not mobile. Instead of a time-consuming visit to the practice, Dr. Masri uses a secure video consultation via the TI Messenger. The nurse transmits current vital signs and photos directly into the chat during the consultation. Dr. Masri adjusts the medication - quickly, safely, and documents.

     

    Advantages: TI-compliant telemedicine enables secure communication. Structured data, such as vital signs or medication plans, can be integrated directly. This is ideal for general practitioners, nursing staff, and mobile services, and reduces travel distances while increasing care security.

  • A GP recognizes an urgent neurological diagnosis in Ms Schneider. Instead of waiting for a callback, he contacts a specialist practice directly via the TI Messenger. Mrs. Schneider receives an appointment on the same day, and the GP can even bill the referral.

     

    Advantages: Fast appointment allocation occurs without lengthy telephone chains. At the same time, flat rates can be charged for appointments, which increases efficiency and patient satisfaction.

  • An emergency paramedic is on his way to the hospital. He uses the TI Messenger to send vital data and photos of the patient directly to the emergency room. The hospital team can make targeted preparations. When the patient arrives, everything is ready - from CT to surgery.

     

    Advantages: Real-time communication between the emergency services and the hospital improves preparation. The handover is seamless and without media disruptions, ensuring optimal care.

  • Mr. Demir lives with diabetes. Every day, he enters his values into an app connected to the TI Messenger. His diabetologist sees the data, gives feedback, and adjusts his medication if necessary - all without a visit to the practice. Mr. Demir feels safe and well looked after.

     

    Advantages: >Structured patient communication is GDPR-compliant and can be integrated into the electronic patient record (EPR). The result is modern, digital care with a personal touch.

Structured Data Exchange for Better Medical Care

In the context of Artificial Intelligence, the importance of the TI messenger in the area of structured data exchange should not be underestimated. Here, the Interoperability Council has firmly planned TIM (TI messenger) and KIM (Communication in Medicine) in order to be able to send data in a structured manner via both secure messaging services in the future. Already, tim+ enables structured data exchange via the FHIR standard. Once the data is available in structured form for service providers and insured persons, it will provide completely new insights into care and treatment processes. 

Patients also benefit considerably from structured data exchange. Especially in the area of patient monitoring, there is still a lot of potential that has not yet been fully exploited in Germany. Once the messenger service has arrived in the patient portals and thus in the living room of the insured, appointment reminders, medication intake or complaints can be documented and received in an uncomplicated manner via tim+ in the future. In particular, automated chatbots will be used to ask patients regularly and in a standardized manner how they are feeling following an operation or a change in medication. The language of the chatbot can be changed to the patient's own language. It is important that the chatbot is seen as a helpful companion and does not replace medical professionals. Use of these functions is always on a voluntary basis.

In the future, several million people will be able to benefit from the advantages and features of the TI messenger. There are no limits to the further development of functionalities. The topic of Artificial Intelligence is not neglected here, and in addition to the increasingly frequent human-to-machine communication, machine-to-machine (M2M) communication is also increasingly coming into the focus of healthcare facilities.

Technical Requirements for the Use of tim+

gematik has announced that in order to use a TI messenger, each institution or service provider organization must have a Security Module Card Type B (SMC-B). An SMC-B is a practice ID card to identify oneself for the use of the telematics infrastructure (TI). Authentication for the TI messenger takes place via OpenID connect or via an existing KIM address. Since doctors' offices and psychotherapists have been required to have TI access since 2019 and outpatient care services and facilities can already connect to the TI, the TI Messenger can already be used if you have a TI connection. The next step is to install the messenger solution in the medical facility. From there, the individual users can be created and managed. The users can decide for themselves whether and how they want to use the TI messenger. An integration of tim+ into the primary systems is just as possible as a separate use via a web or mobile client.

    Currently, the use of a TI messenger is not mandatory. Each medical facility can decide for itself whether or not to use a messenger service.

  • What is the difference between KIM and TIM?

    KIM is a communication service via which healthcare providers can send cross-sector e-mails containing documents such as eAUs (electronic certificates of incapacity for work), digital transfer sheets or findings. A prerequisite for the use of KIM is a connection to the TI through a connector on site or in the data center, whereby the use can take place exclusively in the practice, nursing home or other medical facility. The use of a KIM service is mandatory for all practices that want to send e-doctor letters or eAUs.

    TIM, on the other hand, is a messenger similar to WhatsApp - only it can be used in a legally secure manner in the healthcare sector. The messenger can be used regardless of location and device, which means it can be used flexibly for internal as well as cross-sector communication. Colleagues can easily exchange images and information on the health status of their patients - for example, a wound - or contact the doctor directly via smartphone. There is currently no obligation to use the TI Messenger.
